Toledo 5 "Men in Black III 3D" Contest
HERE'S HOW TO ENTER: Go to the contest page on Toledo5.com. Fill out all appropriate lines of contest entry form. Only one entry per person; additional entries will not be counted. Deadline to enter is Monday, May 21st, 2012 at 12:01 am. You must be 18 years or older to qualify.

Send a postcard entry with entry information via snail-mail to Toledo 5, 115 South Reynolds Road, Toledo, OH 43615, Attn: "Men In Black III 3D” by the deadline date.
WINNER SELECTION: Twenty-five (25) winners will be randomly selected from all eligible entries received on Monday, May 21st, 2012 at 12:01am to receive one pass, admitting two people, to see Men in Black III in 3D at a movie premiere on Tuesday, May 22nd at 7:30pm at Levis Commons Theater in Perrysburg. The winning names will be posted on the Toledo5.com website. Odds of winning are dependent on the number of eligible entries received. Winners will be notified by phone and/or via e-mail. Please provide valid contact information for notification. Winners will be chosen on a weekly basis.
PRIZES & APPROXIMATE RETAIL VALUES: Each winner will receive one pass, admitting two persons, to see Men in Black III in 3D at a movie premiere on Tuesday, May 22nd at 7:30pm at Levis Commons Theater in Perrysburg. Value for passes is approximately $25.
GENERAL RULES: To enter, you must be age 18 or older to enter. Open only to residents of Ohio and Michigan. No purchase necessary. Employees of CBS, Time-Warner, Viacom, Warner Bros., The CW, WT05, Block Communications, The Owens Group, participating radio stations, participating TV stations, their affiliates, subsidiaries, advertising and promotion agencies and their immediate family members and/or those living in the same household of each are not eligible. No mechanically reproduced entries permitted. Void where prohibited by law. Decisions of judges are final. Not responsible for lost, late, mutilated, misdirected entries or entries not received in time for the random drawing(s). Prize winners may be required to pick up prize from the Toledo 5 offices.
No substitution or transfer of prize permitted. All federal, state and local taxes are the sole responsibility of winner. All federal, state and local laws apply. Prizes consist of only the items specifically listed as part of the prize. Potential Grand Prize winner must sign and return an Affidavit of Eligibility/Release of Liability/Prize Acceptance Form within (3) days of attempted notification. Noncompliance within this time period may result in disqualification and an alternate may be selected. Return of any prize/prize notification as undeliverable will result in disqualification. All travelers must execute a Release of Liability prior to ticketing and must possess required travel documents (e.g. valid photo I.D.) prior to departure.
If a minor wins the Grand Prize, prize will be awarded in the name of a parent or legal guardian who must accompany minor on trip. Departure and return times are subject to change. WT05 reserves the right to substitute prize of equal or greater value. Acceptance of prize constitutes permission to the sponsor and its agencies to use winner's name and/or likeness for purposes of advertising and trade without further compensation, unless prohibited by law. By accepting prize, winners agree to hold sponsors, their respective directors, officers, employees and assigns, harmless against any and all claims and liability arising out of use of prize.
Winners assume all liability for any injury or damage caused, or claimed to be caused, by participation in this promotion or use or redemption of any prize. By participating in this promotion, entrants agree to be bound by the Official Rules and the decisions of the judges. Sponsors not responsible for any typographical or other error in the printing of the offer, administration of the sweepstakes or in the announcement of prizes. Only one winner per household. Winners are only eligible for one grand prize per month. WT05 is not responsible for delivering prize if promoter cancels event.
