Toledo 5 Summer Survival Contest
HERE'S HOW TO ENTER: Go to the contest page at toledo5.com and completely fill out the on-line entry form for a chance to win the listed prizes.
Only one entry per person; additional entries will not be counted. Deadline to enter is Tuesday, May 29th at 12:01am. You must be 18 years or older to qualify.

WINNER SELECTION: Five (5) winners will be randomly selected from all eligible entries to the online contest. The deadline to enter is Tuesday, May 29th at 12:01am. Odds of winning are dependent on the number of eligible entries received. Only one entry per person; additional entries will not be counted.
PRIZES & APPROXIMATE RETAIL VALUES: Five (5) winners will win a prize pack consisting of two (2) tickets to see The Counting Crows in concert at Centennial Terrace on June 16th, two (2) tickets to see Gavin De Graw and Colbie Caillat in concert at Centennial Terrace on August 1st, and a tote bag filled with CW and Toledo 5 prizes. Total value per prize pack is approximately $150.
GENERAL RULES: Must be age 18 or older to enter. Open only to residents of Ohio and Michigan. No purchase necessary. Employees of CBS, Time-Warner, Viacom, Warner Bros., The CW, WT05, Block Communications, The Owens Group, participating radio stations, participating TV stations, their affiliates, subsidiaries, advertising and promotion agencies and their immediate family members and/or those living in the same household of each are not eligible. No mechanically reproduced entries permitted. Void where prohibited by law. Decisions of judges are final. Not responsible for lost, late, mutilated, misdirected entries or entries not received in time for the random drawing(s). Prize winners may be required to pick up prize from the Toledo 5 offices.
No substitution or transfer of prize permitted. All federal, state and local taxes are the sole responsibility of winner. All federal, state and local laws apply. Prizes consist of only the items specifically listed as part of the prize. Potential Grand Prize winner must sign and return an Affidavit of Eligibility/Release of Liability/Prize Acceptance Form within (3) days of attempted notification. Noncompliance within this time period may result in disqualification and an alternate may be selected. Return of any prize/prize notification as undeliverable will result in disqualification.
WT05 reserves the right to substitute prize of equal or greater value. Acceptance of prize constitutes permission to the sponsor and its agencies to use winner's name and/or likeness for purposes of advertising and trade without further compensation, unless prohibited by law. By accepting prize, winners agree to hold sponsors, their respective directors, officers, employees and assigns, harmless against any and all claims and liability arising out of use of prize.
Winners assume all liability for any injury or damage caused, or claimed to be caused, by participation in this promotion or use or redemption of any prize. By participating in this promotion, entrants agree to be bound by the Official Rules and the decisions of the judges. Sponsors not responsible for any typographical or other error in the printing of the offer, administration of the sweepstakes or in the announcement of prizes. Only one winner per household. Winners are only eligible for one grand prize per month. WT05 is not responsible for delivering prize if promoter cancels event.
