Toledo 5 Tony Roberts - Toledo Funny Bone Contest
HERE'S HOW TO ENTER: Go to the contest page on Toledo5.com and locate the entry blank. Fill out all appropriate lines of contest entry form. Only one entry per person, additional entries will not be counted. Deadline to enter is Wednesday, January 30th, 2013 at 12:01 am. You must be 21 years or older to qualify.
WINNER SELECTION: One (1) winner will be randomly selected from all eligible entries received by Wednesday, January 30th, 2013 at 12:01am to receive tickets. The winning name will be posted on the Toledo5.com website. Odds of winning are dependent on the number of eligible entries received. The winner will be notified by phone and/or via e-mail. Please provide valid contact information for notification. Tickets will be available at Toledo5’s Office located on 115 S. Reynolds Rd., Toledo, OH 43615. The winner must be able to receive tickets from this location by Friday, February 1 at 4:00pm or the prize will be forfeited.
PRIZES & APPROXIMATE RETAIL VALUES: 1 winner will receive 4 tickets to the Tony Roberts Comedy Show at the Toledo Funny Bone on February 2, 2013 @ 7pm, located at 6140 Levis Commons Blvd Perrysburg, OH 43551; visit www.funnybonecentral.com for a complete schedule. Tickets are valued at $22 each.
GENERAL RULES: To enter, you must be age 21 or older to enter. Open only to residents of Ohio and Michigan. No purchase necessary. Employees of CBS, Time-Warner, Viacom, Warner Bros., Carsey-Werner, The CW, Toledo 5, Block Communications, The Owens Group, participating radio stations, participating TV stations, their affiliates, subsidiaries, advertising and promotion agencies and their immediate family members and/or those living in the same household of each are not eligible. No mechanically reproduced entries permitted. Void where prohibited by law. Decisions of judges are final. Not responsible for lost, late, mutilated, misdirected entries or entries not received in time for the random drawing(s). Prize winners may be required to pick up prize from the Toledo 5 offices.
No substitution or transfer of prize permitted. All federal, state and local taxes are the sole responsibility of winner. All federal, state and local laws apply. Prizes consist of only the items specifically listed as part of the prize. Potential Grand Prize winner must sign and return an Affidavit of Eligibility/Release of Liability/Prize Acceptance Form within (3) days of attempted notification. Noncompliance within this time period may result in disqualification and an alternate may be selected. Return of any prize/prize notification as undeliverable will result in disqualification. All travelers must execute a Release of Liability prior to ticketing and must possess required travel documents (e.g. valid photo I.D.) prior to departure.
If a minor wins the Grand Prize, prize will be awarded in the name of a parent or legal guardian who must accompany minor on trip. Departure and return times are subject to change. Toledo 5 reserves the right to substitute prize of equal or greater value. Acceptance of prize constitutes permission to the sponsor and its agencies to use winner's name and/or likeness for purposes of advertising and trade without further compensation, unless prohibited by law. By accepting prize, winners agree to hold sponsors, their respective directors, officers, employees and assigns, harmless against any and all claims and liability arising out of use of prize.
Winners assume all liability for any injury or damage caused, or claimed to be caused, by participation in this promotion or use or redemption of any prize. By participating in this promotion, entrants agree to be bound by the Official Rules and the decisions of the judges. Sponsors not responsible for any typographical or other error in the printing of the offer, administration of the sweepstakes or in the announcement of prizes. Only one winner per household. Winners are only eligible for one grand prize per month. Toledo 5 is not responsible for delivering prize if promoter cancels event.
